Chipped Foundation_Blue Foam Exposed

Hi all:

This will be the first of many posts here. My wife and I just bought our first house and unfortunately it is a money pit..everyday we find something else wrong.

This spring, after the snow melted, I noticed a number of areas on the exposed foundation where the thin concrete layer had chipped off, exposing blue foam insulation.

2 questions: 1. What causes this; and 2. what do I do to fix it?

was your house foundation constructed of styrofoam foundation forms? that means to say the forms are a part of the foundation [cells]

No clue. House wa sbuilt 20 years ago. The chipped concrete is a very thin layer covering the blue foam.
