Leviton 5225 wiring diagram

I can't find a proper wiring diagram for this switch/receptacle anywhere...

I've got everything hooked up according to diagram #31 here: http://www.lightingfacts.com/Recepta.../%20Receptacle

But, the switch is still controlling the receptacle. I'm guessing that the diagram is based on another type of switch. Does anyone know how to wire the Leviton 5225, or know where I can get the wiring diagram?

This is great! I searched high and low for the way to wire this switch/outlet combo. Thank you for the diagram HayZee. I used it and it worked perfectly.
